Duplicate Licence
Duplicate driving licence applications must be accompanied by:
Application form (D800) fully completed and signed and stamped at your local Garda station.
Two recent passport type photographs signed on the back by the applicant.
Where the applicant requires a duplicate licence to be issued in a name other than that on the original licence, supporting documentation e.g. original marriage certificate or original deed poll must be submitted.
Fee - 10 EUR
