Duplicate Licence

Duplicate driving licence applications must be accompanied by:

Application form D401 (full licence) or D201 (learner permit) fully completed in BLACK pen and signed and stamped at your local Garda station.
Two recent passport type photographs signed on the back by the applicant.
Where the applicant requires a duplicate licence to be issued in a name other than that on the original licence, supporting documentation e.g. original marriage certificate or original deed poll must be submitted.
Fee - €35.00
